Note from EBISD Nurse's Office Regarding Forms for Students Care At EBISD

Dear parents, in preparation for coming school year please find the forms linked below for your physician to fill out with regards your student's care at school.

EBISD Student Care Forms

Any student with an allergy that requires medical intervention with epi-pen at school please have MD complete Anaphylaxis Care Plan.

Any student with diagnosis Seizure disorder please have MD complete Seizure action plan.

Any student fifth through twelfth grade that requires an inhaler and wants to carry with them please have MD fill out School asthma action plan with self carry component.Students PreK-5  will have their inhaler in nurse office.

If not completed prior to school forms will be sent home with student and need to be completed as soon as possible. To help ease stress with back to school getting with your primary care provider prior to this time is recommended. 

Any medication to be administered at school needs to be sent in original container with medication administration form completed. Medication admin form also on website .

STUDENTS CAN NOT BRING MEDICATION TO OR FROM SCHOOL . 

PARENT/CAREGIVER  WILL NEED TO BRING TO NURSE OFFICE
